License by Examination

A year long LPN program prepares the individual to sit for the licensure test- NCLEX-PN. Before the completion of the course, the candidates must submit an application for the test. For this, the candidates need to submit a duly signed 2×2 passport size photograph and the application along with a money order of $100 (payable to "Louisiana State Board of Practical Nurse Examiners") at the given-below address:

The candidate should submit all the relevant documents including the ones related to arrest in the past (if any). Simultaneously, the candidate must register with the licensing agency (responsible for conducting the NCLEX-PN). The registration can be done by either of the following methods.

Online: A fee of $200 has to be paid via Master Card.

Or

Via Mail: The candidate can request the Board of Nursing to send an NCLEX-PN registration form via mail. Fill the details and send it to the board along with a money order of $200 (payable to the National Council of State Boards of Nursing) at the following address:

On verifying your documents, the board will inform the testing agency about your eligibility for taking the licensure test. The eligible candidates will receive an ATT (Authorization to Test) through which, they can schedule the exam date and location.
On the exam day, don’t forget to take your ATT and a valid ID proof. If the candidate is unable to clear the licensure test in the first attempt, s/he can appear for it again after 45 days.

License by Endorsement

Those LPNs, who hold an active license in another state and want to get transferred to Louisiana and continue the practice, can get the LPN license in Louisiana through endorsement.

  • The candidates must fill the application and send it to the board along with a copy of the current license, fingerprint and background check report.

The fee charged for endorsement application is $100. An additional fee of $18 is to be paid for the background check.

  • Louisiana nursing license verification is carried out to check the authenticity of the license. The nursing board requires the LPN to request a transcript of the course from the original nursing school and submit its copy. The transcript serves as an eligibility to get the license. The transcript must include number of clock hours of the course and other necessary information. The board will then verify if the LPN course studied by the nurse fulfills the demands of the Louisiana nursing board's coursework.
  • The board may issue a temporary work permit to the nurses on receiving all the relevant documents. If it has been four or more years since the candidate passed the course, the board checks that the candidate holds a working experience in the last six months. If it is not so, the aspirant is required to take a refresher course.

Louisiana Nurse Aide Registry maintains the records of all those nurses who have been licensed in the state. It aids in verifying the status of the license.

Louisiana Nursing License Renewal

The license has to be renewed after every 2 years. The nurse practicing with an expired license is punishable by the law. It is better to apply for license renewal before the date of expiration. The nurses are reminded of the renewal by the board through mail, a few days before the date of expiration.

  • In order to renew the license online, the candidates need to register online and provide the LPN license number, date of birth, and four digits social security number. A valid email address would be required to register as a user.
  • The candidates are required to fill the online application and submit a proof of 5 contact hours and 16 nursing hours of education. If the nursing hours are falling short, it can be compensated by increasing the number of contact hours.
  • Once you have filled the online application and mailed it to the board with the relevant documents, it might take a minimum of 14 days for the board to send the license through mail. All the licenses issued in 2017 would be paperless.
  • The fee charged for renewal is $60. If you apply for license renewal after the expiry date, you may have to pay an additional fee of $70, which makes the total cost to be $130.

License Reinstatement

The LPNs who want to reinstate their license that has lapsed due to non-renewal or suspended for violating the laws of the nursing board can get the license activated by fulfilling the following conditions:

  • The candidate must ensure that s/he is clear of all the charges that lead to the suspension of the license.
  • The candidate must submit a written application requesting for reinstatement of the license.
  • If the board requires a chemical and physiological assessment to be done before reinstatement, then the person who conducted the test must submit a letter of approval stating that the person is fit enough to practice the profession. If the assessment involves a treatment plan, then a proof of compliance with the proposed plan must also be provided.

The board reviews the request, and if it finds the documents appropriate, it reinstates the license for two years. The practical nurse must submit a fee of $500.00 through a money order. Failure to deposit the fees within the limited time will result in suspension of the license.

Duplicate License

If a nurse loses the license or it gets destroyed or tampered to the extent that it can't be used again, the nurses can request a duplicate license by submitting an application to the nursing board. The following documents should be mailed along with the application.

  1. A copy of the driver's license.
  2. The board does not accept checks. Therefore, a money order of $30.00 (payable to "LSBPNE") should be sent to the board.
  3. Original birth certificate that is certified (The board keeps the photocopy and returns the original to the nurse with duplicate LPN license)
